No. The $10 fee applies only to replacement of forms that have already been issued. New requests are assessed during a consultation.

How long does it take to receive a replacement form?
Replacement forms are processed as soon as possible and may take up to one business day, depending on clinical availability.

Do I need to show identification when collecting the form?
Yes. Identification is required to protect your privacy and ensure documents are released to the correct patient.

When do I need to pay the fee?
Payment is required before the replacement form is released. For your convenience, EFTPOS is available onsite. Please note that cash payments are not accepted.
